PCV – BUS & MINIBUS MEDICALS
A D4 medical is required of all PCV and bus drivers when applying for a provisional category D licence.
After the initial medical you need a renewal medical at the age of 45, which is obligatory every 5 years until it becomes an annual requirement at 65.
Notes about Minibus Medicals
Did you pass your driving test after 1997? If yes then you will require a D4 medical in order to obtain a provisional D1 licence – if you want to drive a minibus over 3.5 tonne (commercially or otherwise).
What is involved in the D4 medical?
– The eyesight test
– Blood pressure checks
– Questionnaire regarding your medical history
What to bring to your D4 medical:
• The D4 Medical Form
• Photo ID (e.g. photo driving license or passport)
• Your driving glasses or contact lenses and the prescription
• Details of any ongoing medical conditions plus any medication you are currently taking
Please note that it is your responsibility to check that it is acceptable for a doctor other than your own GP to complete the form with the licensing authority.
